Category AOfficial Undergraduate Transcript |
An official and sealed UNDERGRADUATE transcript, which states that a degree has been awarded or conferred. The name on the transcript must match your current legal name or proof of name change is required * International Students: This cannot be a diploma. It must be an official and sealed English translation which lists all the courses taken and grades received. |
Category B Official Graduate or Post-Graduate Transcript |
An official and sealed Graduate or Post-Graduate transcript may be submitted. All graduate work must have been completed. The transcript must state that a degree has been awarded or conferred. The name on this transcript must match your current legal name or proof of name change will be required.* |
Category C Official Seattle University Transcripts |
An official and sealed Seattle University transcript may be submitted, but ONLY if you have studied at least one quarter in on-site classes at Seattle University.. You must be a current Seattle University graduate student to qualify for this category. The name on this transcript must match your current legal name or proof of name change will be required. * |
* Proof of Name Change - The name on your Seattle University Certificate request form and/or S-TESL certificate request form must match the name on the transcript you are submitting as proof of your degree. If not, include proof of name change.
Acceptable forms of proof of name change are:
- a copy of your marriage certificate
- a copy of your legal name change document
- a copy of your divorce certificate (if name change is from married to maiden)
